To Enter: Viewers watch "Wild About Houston" on Fridays at 1 p.m. and Sundays at 9 p.m. November 14 – November 20, 2008 During the Wild About Houston viewers will be asked to log onto www.houstons55.com and register to win one (1) of twenty (20) family four packs to the movie Madagascar by filing out and submitting required online form. ** All online entries must contain: name, address, daytime telephone number, evening telephone number, (if different from daytime telephone number) email address and birth date. 2. Eligibility Restrictions: A. This contest is open to all viewers who are U.S. residents and 18 years of age or older. Employees of KTBU, USFR Media, and Levinson and Hill, their affiliates & subsidiaries, immediate family members, employees of other Texas area television stations and their families are ineligible to participate in the contest. Winners must have a valid social security card and a valid government or state issued photo id. B. Viewers are eligible to win a KTBU contest only once every sixty days. Viewers are eligible to win a prize valued at $600 or more once every six months. Only one winner per household is permitted in any contest. 3. Prize: A. KTBU will be award twenty (20) winners one (1) family (4) pack for the movie Madagascar: Escape 2 Africa. Each winner will receive one (1) family four (4) pack which includes: entry for four (4) to predestinated theater where Madagascar: Escape 2 Africa is showing and two (2) free dink and popcorn vouchers. Theater tickets are only valid at theater listed on ticket and will be valid through full run of Madagascar: Escape 2 Africa. The tickets and vouchers are nonrefundable, nontransferable and not redeemable for cash, alcoholic beverages, and gratuity. Other Restrictions may apply. All additional costs, including but not limited to parking, beverages, fees, taxes, tips, travel to and from location, are the winner's responsibility. B. Eligible contestants will have the opportunity to qualify to win by logging onto www.houstons55.com and filling out a registration form beginning at 1 p.m. Friday, November 14, 2008 and ending at 10am Thursday, November 20, 2008, KTBU reserves the right to extend the end date of this contest upon prior notification to viewers. 4. Selection of winners: A. Decisions of station management with respect to the contest are final. B. Winners will be randomly drawn from all entries by a KTBU executive on Thursday, November 20, 2008. Winners' name will be posted on www.Houstons55.com on "Wild About Houston" home page beginning Friday, November 21, 2008 1 p.m. C. Winner will be notified via e-mail and /or telephone on Thursday, November 20, 2008, and will have ten (10) business days from day of notification to claim prize by contacting KTBU Offices via email and/or telephone between the hours of 9 a.m. and 4 p.m. at which point winner will need to schedule a time to pick up prize and bring required identification for photocopy to KTBU Channel 55. If winner fails to claim prize within the allotted time, the prize will be forfeited. KTBU is not responsible for any problems which winner may incur while trying to claim the prize. KTBU will confirm the identity of the winner and his/her information upon contact. D. Release forms and prize must be picked up by scheduling an appointment during business hours, Monday through Friday, 9 a.m.-5:30 p.m. at the office of KTBU Channel 55, 11150 Equity Drive, Houston, Texas 77041 within 30 days from the date of winning. Prize materials not claimed by this date will be forfeited. 5. Conditions: A. Payment of all federal, state and local taxes is the sole responsibility of the winner. Winners will receive an IRS Form W-9 or the equivalent upon receipt of prize. B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and sign a publicity release prior to acceptance of this prize. C. Contest winners will be required to sign a liability release agreeing to hold KTBU and "Stacy Chapman Salon" their owners, corporate licensee and its parent, subsidiary and affiliated corporations harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. 6. KTBU reserves the right to make changes in the rules of the contest, which will become effective upon announcement. 7. Failure to comply with the contest rules may result in a contestant's disqualification. 8. KTBU is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or the announcements of the prize. 9. KTBU is not responsible for any problems the participant may incur while trying to mail or email entry nor are they responsible for any entry, which may be received after contest deadlines. 10.
