To Enter: <br />Beginning Feb. 15th, 2010 through March 15th, 2010, viewers will be asked to log onto www.houstons55.com and register to win one (1) of twelve (12) It’z Family Packs, each pack includes 4 Free Buffets, 4 Free Drinks, and 4 $5 Game Cards. Value of each Family pack is $50. Three (3) winners will be drawn on Feb. 22nd, March 1st, March 8th, and March 15th, 2010.<br />Entry is easy, simply register at Houstons55.com, fill out and submit the required online form.<br /><br />** All online entries must contain: name, address, daytime telephone number, evening telephone number, (if different from daytime telephone number) email address and birth date.<br /><br />2. Eligibility Restrictions: <br />A. This contest is open to all viewers who are U.S. residents and 18 years of age or older. Employees of KTBU, USFR Media, and IT’Z Family Food & Fun/ Allied Integrated Mktg., and their affiliates & subsidiaries, immediate family members, employees of other Texas area television stations and their families are ineligible to participate in the contest. Winners must have a valid social security card and a valid government or state issued photo id.<br />B. Viewers are eligible to win a KTBU contest only once every sixty days. Viewers are eligible to win a prize valued at $600 or more once every six months. Only one winner per household is permitted in any contest. <br /><br />3. Prize: <br /><br />KTBU will be awarding twelve (12) It’z Family Packs, each pack includes 4 Free Buffets, 4 Free Drinks, and 4 $5 Game Cards courtesy of IT'Z Family Food & Fun/ Allied Integrated Mktg. All additional costs, including but not limited to parking, fees, taxes, tips, travel to and from location, are the winner’s responsibility. <br /><br />Eligible contestants will have the opportunity to qualify to win by logging onto www.houstons55.com, filling out a registration form beginning at 11 a.m. Feb. 15th, 2010 and ending at 8 a.m. March 15th, 2010, KTBU reserves the right to extend the end date of this contest upon prior notification to viewers. <br /><br /><br /><br /><br />4. Selection of winners:<br /><br />Decisions of station management with respect to the contest are final. <br /><br />Three winners will be randomly drawn on Feb. 22nd, March 1st, March 8th, and March 15th, 2010.from all entries by a KTBU executive. Winners’ name will be posted on www.Houstons55.com contest page beginning Feb. 22nd, 2010 at 1 p.m. <br /><br />Winners will be notified via e-mail and /or telephone on Feb. 22nd, 2010, and will have ten (10) business days from day of notification to claim prize by contacting KTBU Offices via email and/or telephone between the hours of 9 a.m. and 4 p.m. at which point winner will need to schedule a time to pick up prize and bring required identification for photocopy to KTBU Channel 55. If winner(s) fails to claim prize within the allotted time, the prize will be forfeited. KTBU is not responsible for any problems which winner may incur while trying to claim the prize. KTBU will confirm the identity of the winner and his/her information upon contact. <br /><br />Release forms and prize must be picked up by scheduling an appointment during business hours, Monday through Thursday, 9 a.m.-4:30 p.m. and Friday’s 9 a.m.-3 p.m. at the office of KTBU Channel 55, 11150 Equity Drive, Houston, Texas 77041 within 30 days from the date of winning. Payment of all federal, state and local taxes is the sole responsibility of the winner. Winners will receive an IRS Form W-9 or the equivalent upon receipt of prize. <br />B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and sign a publicity release prior to acceptance of this prize. <br />C. Contest winners will be required to sign a liability release agreeing to hold KTBU and their owners, corporate licensee and its parent, subsidiary and affiliated corporations harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. <br /><br />6.
