Rules:KTBU Houston’s 55 Macy’s Giftcard Giveaway Contest Dates: August 3 through August 17, 2009 NO PURCHASE NECESSARY. VOID WHERE PROHIBITED 1. To Enter: Viewers who watch Houston’s 55 beginning Thursday, August 3, 2009 through Monday, August 17, 2009 will be asked to log onto www.houstons55.com and register to win one (1) of two (2) two hundred and fifty dollar ($250.00) Macy’s Gift cards by filing out and submitting required online form. One entry per household All online entries must contain: name, address, daytime telephone number, evening telephone number, (if different from daytime telephone number) email address and birth date. 2. Eligibility Restrictions: A. This contest is open to all viewers who are U.S. residents and 18 years of age or older. Employees of KTBU, USFR Media, and Macy’s, their affiliates & subsidiaries, immediate family members, employees of other Texas area television stations and their families are ineligible to participate in the contest. Winners must have a valid social security card and a valid government or state issued photo id. B. Viewers are eligible to win a KTBU contest only once every sixty days. Viewers are eligible to win a prize valued at $600 or more once every six months. Only one winner per household is permitted in any contest. 3. Prize: A. KTBU will be awarding two (2) winners, each one (1), two hundred and fifty ($250.00) dollar Macy’s gift card. Two (2) winners will receive one (1) gift card, each gift card is valid for two hundred and fifty ($250.00) dollars and may be redeemed at your local Macy’s. Gift Cards are nonrefundable, nontransferable, not redeemable for cash, not valid with any other offers and or special events. Other restrictions may apply. All additional costs are the winner’s responsibility. B. Eligible contestants will have the opportunity to qualify to win by logging onto www.houstons55.com and filling out a registration form beginning Monday, August 3, 2009 and ending at 10 a.m. Monday, August 17, 2009, KTBU reserves the right to extend the end date of this contest upon prior notification to viewers. 4. Selection of winners: A. Decisions of station management with respect to the contest are final. B. One winner will be randomly drawn from all entries by a KTBU executive Monday, August 10, 2009 and another Monday, August 17,2009. Winners’ name will be posted on www.Houstons55.com on contest page beginning, Tuesday August 11, 2009 1 p.m. C. Winner will be notified via e-mail and /or telephone on Tuesday, August 10 and Tuesday 18, 2009, and will have until close of business the day tickets must be used to claim prize Weekend and Holiday winners will be notified on the following regular business day. The prize may be claimed by visiting KTBU offices Monday - Friday between 8:30 a.m. and 5:30 p.m. If winner fails to claim prize within the allotted time, the prize will be forfeited. KTBU is not responsible for any problems which winner may incur while trying to claim the prize. KTBU will confirm the identity of the winner by KTBU will require and make a copy of winner’s government or state issued photo ID upon claiming prize. Winner will also be required to fill out a receipt of award form and a W9. D. Documents necessary to receive award may be obtained at the offices of KTBU Channel 55, 11150 Equity Drive, Houston, Texas 77041, Monday through Friday, 8:30 a.m.-5:30 p.m. within 30 days from the date of winning. Prize materials not claimed by this date will be forfeited 5. Conditions: A. Payment of all federal, state and local taxes is the sole responsibility of the winner. Winners will receive an IRS Form W-9 or the equivalent upon receipt of prize. B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and sign a publicity release prior to acceptance of this prize. C. Contest winners will be required to sign a liability release agreeing to hold KTBU and contest giveaway partner , corporate licensee and its parent, subsidiary and affiliated corporations harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. 6. KTBU reserves the right to make changes in the rules of the contest, which will become effective upon announcement. 7. Failure to comply with the contest rules may result in a contestant’s disqualification. 8. KTBU is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or the announcements of the prize. 9. KTBU is not responsible for any problems the participant may incur while trying to mail or email entry nor are they responsible for any entry, which may be received after contest deadlines. 10.
