VOID WHERE PROHIBITED 1.To Enter: Houston’s 55 viewers will be instructed to log onto www.houstons55.com where they may register to win one (1) of three (3) ninety nine ($99.00) electronic tooth brushes to be awarded every other week beginning Monday, July 13 to Friday August 21, 2009. ** All emailed entries must contain: name, address, daytime telephone number, evening telephone number, (if different from daytime telephone number) email address and birth date. 2. Eligibility Restrictions: A. This contest is open to all viewers who are U.S. residents and 18 years of age or older. Employees of KTBU, USFR Media and Town and Country Dental, their affiliates & subsidiaries, immediate family members, employees of other Texas area television stations and their families are ineligible to participate in the contest. Winners must have a valid social security card and a valid government or state issued photo id. B. Viewers are eligible to win a KTBU contest only once every sixty days. Viewers are eligible to win a prize valued at $600 or more once every six months. Only one winner per household is permitted in any contest. 3. Prize: A. KTBU will award three (3) ninety nine dollar ($99.00) electronic toothbrushes once every other week to one (1) lucky winner. Winners will receive one (1) ninety nine ($99.00) dollar electronic tooth brush. Tooth brush is nonrefundable, nontransferable, not redeemable for cash, and not valid with any other offers. Other restrictions may apply. All additional costs, including but not limited to fees, taxes, tips, are the winner’s responsibility. B. Eligible contestants will have the opportunity to qualify to win by going to www.houstons55.com and registering to win beginning Monday, July 13, 2009, and ending Friday, August 21, 2009, KTBU reserves the right to extend the end date of this contest upon prior notification to viewers. 4. Selection of winners: A. Decisions of station management with respect to the contest are final. B. Winner will be randomly drawn daily (Monday-Friday) from all entries by a KTBU executive beginning Tuesday, July 17, 2009. Winners’ name will be posted on www.houstons55.com on contest page beginning, Tuesday, July 17, 2009 after 1 p.m. C. Winner will be notified via e-mail and /or telephone on Monday, July 17, 2009, and will have and will have 30 days from date prize was awarded to claim prize. The prize may be claimed by visiting KTBU offices Monday - Friday between 8:30 a.m. and 5:30 p.m. If winner fails to claim prize within the allotted time, the prize will be forfeited. KTBU is not responsible for any problems which winner may incur while trying to claim the prize. KTBU will confirm the identity of the winner by KTBU will require and make a copy of winner’s government or state issued photo ID upon claiming prize. Winner will also be required to fill out a receipt of award form and a W9. D. Prize materials not claimed by this date will be forfeited 5. Conditions: A. Payment of all federal, state and local taxes is the sole responsibility of the winner. Winners will receive an IRS Form W-9 or the equivalent upon receipt of prize. B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and sign a publicity release prior to acceptance of this prize. C. Contest winners will be required to sign a liability release agreeing to hold KTBU and “Stacy Chapman Salon” their owners, corporate licensee and its parent, subsidiary and affiliated corporations harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. 6. KTBU reserves the right to make changes in the rules of the contest, which will become effective upon announcement. 7. Failure to comply with the contest rules may result in a contestant’s disqualification. 8. KTBU is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or the announcements of the prize. 9. KTBU is not responsible for any problems the participant may incur while trying to mail or email entry nor are they responsible for any entry, which may be received after contest deadlines. 10.
