To Enter: Viewers who watch Houston’s 55 beginning Monday, July 6, 2009, through Friday, July 31, 2009 will be instructed to log onto www.houstons55.com visit the contest page and register to win one (1) of three (3) Family gift Package to San Antonio, Texas valued at an estimated one thousand four hundred ($1400.00) dollars each by filing out and submitting required online form. All online entries must contain: name, address, daytime telephone number, evening telephone number, (if different from daytime telephone number) email address and birth date. 2. Eligibility Restrictions: A. This contest is open to all viewers who are U.S. residents and 18 years of age or older. Employees of KTBU, USFR Media, and San Antonio Conventions and Visitors Bureau, their affiliates & subsidiaries, immediate family members, employees of other Texas area television stations and their families are ineligible to participate in the contest. Winners must have a valid social security card and a valid government or state issued photo id. B. Viewers are eligible to win a KTBU contest only once every sixty days. Viewers are eligible to win a prize valued at $600 or more once every six months. Only one winner per household is permitted in any contest. 3. Prize: A. KTBU will be award three (3) winners each, one (1) Family four (4) pack of gift certificates valued at an estimated one thousand and four hundred ($1400.00) dollars. Each Family four pack includes: 1 Complimentary three (3) night stay at either The Omni La Mansion del Rio Hotel valid through 1/31/2010, The Hyatt Place San Antonio Riverwalk valid through 12/30/2009 or The Radisson Downtown Market Square valid through 11/17/2009, four (4) tickets to SeaWorld San Antonio valid thru 12/31/09 , four (4) Six Flags Fiesta Texas valid through 1/3/2010, four (4) ticket to Schlitterbahn New Braunsfels valid through 9/19/2009, four (4) tickets to Splashtown San Antonio valid for the 2009 season, four (4) tickets to the Tower of the Americas and Skies Over Texas 4D ride valid on a space available basis, each package will also include four (4) tickets of to either of the following attractions: San Antonio Botanical Gardens, Enchanted Spring Ranch or Ripley’s Believe It or Not and Louis Tussands Wax Works. All gift certificates are nonrefundable, nontransferable and not redeemable for cash. Other Restrictions may apply. Hotel stay is valid for one room per night , standard room and tax only, no incidentals included, must present vouchers upon check in, reservation must be made two weeks prior to arrival and must be made directly with above mentioned property, not valid New Year’s Eve, Fiesta, Riverlighting and blackout dates as deemed by each individual hotel. Winner is responsible for making specific arrangements as outlined in the individual prizes included in the prize package. Eligible contestants will have the opportunity to qualify to win by logging onto www.houstons55.com and filling out a registration form beginning at 9am Monday, July 6, 2009 and ending at 12 p.m. Friday, July 31, 2009, KTBU reserves the right to extend the end date of this contest upon prior notification to viewers. 4. Selection of winners: A. Decisions of station management with respect to the contest are final. B. Winner will be randomly drawn from all entries by a KTBU executive Friday, July 17th, 24th and 31st, 2009. Winners’ name will be posted on www.Houstons55.com contest page beginning Friday, July 17, 2009 and every Friday thereafter through end of contest. A. Each winner will be notified via e-mail and /or telephone on Friday, July 17th, 24th and 31st, 2009, and will have 30 days from date prize was awarded to claim prize. Weekend and Holiday winners will be notified on the following regular business day. The prize may be claimed by visiting KTBU offices Monday - Friday between 8:30 a.m. and 5:30 p.m. If winner fails to claim prize within the allotted time, the prize will be forfeited. KTBU is not responsible for any problems which winner may incur while trying to claim the prize. KTBU will confirm the identity of the winner by KTBU will require and make a copy of winner’s government or state issued photo ID upon claiming prize. Winner will also be required to fill out a receipt of award form and a W9. B. Documents necessary to receive award may be obtained at the offices of KTBU Channel 55, 11150 Equity Drive, Houston, Texas 77041, Monday through Friday, 8:30 a.m.-5:30 p.m. within 30 days from the date of winning. Prize materials not claimed by this date will be forfeited. 5. Conditions: A. Payment of all federal, state and local taxes is the sole responsibility of the winner. Winners will receive an IRS Form W-9 or the equivalent upon receipt of prize. B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and sign a publicity release prior to acceptance of this prize. C. Contest winners will be required to sign a liability release agreeing to hold KTBU, corporate licensee and its parent, subsidiary and affiliated corporations harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. 6. KTBU reserves the right to make changes in the rules of the contest, which will become effective upon announcement. 7. Failure to comply with the contest rules may result in a contestant’s disqualification. 8. KTBU is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or the announcements of the prize. 9. KTBU is not responsible for any problems the participant may incur while trying to mail or email entry nor are they responsible for any entry, which may be received after contest deadlines. 10.
